What is Infertility?

Infertility is defined as the inability of a married, sexually active couple to achieve pregnancy within one year of unprotected regular intercourse. Infertility may be diagnosed prior to one year if there are features or findings indicative of subfertility.

These include -

  • Oligo or amenorrhoea.
  • Inability to have intercourse.
  • Previous adjuvant therapy for cancer in either partner.
  • History indicating an increased risk of fallopian tube occlusion (that is, previous pelvic infection or previous pelvic surgery).
  • Advanced women age (>35 years).
  • Oligozoospermia or azospermia.

Laparoscopic Fertility Surgery

Laparoscopic fertility surgery is a minimally invasive surgical procedure that can be used to diagnose and treat certain fertility issues that cannot be addressed through other means. During a laparoscopic procedure, a small incision is made near the navel and a thin, lighted tube called a laparoscope is inserted into the abdomen. The laparoscope allows the surgeon to view the reproductive organs and surrounding structures and perform any necessary surgical procedures.

In Vitro Fertilisation (IVF)

In vitro fertilization (IVF) refers to a procedure designed to overcome infertility and produce a pregnancy as a direct result of the intervention. In general, the ovaries are stimulated by a combination of fertility medications and then one or more oocyte(s) are aspirated from ovarian follicles. These are fertilized in the laboratory ("in vitro"), after which, one or more embryo(s) are transferred into the uterine cavity with the intent to establish a successful pregnancy.These steps occur over about a two-week interval of time, which is called an IVF cycle. IVF is a major treatment in infertility when other methods of Assisted Reproductive Technology have failed.

Intracytoplasmic Sperm Injection (ICSI)

Intracytoplasmic sperm injection (ICSI), is an in vitro fertilization procedure in which a single sperm is injected directly into an egg and is used to overcome male infertility problems, although it may also be used where eggs cannot easily be penetrated by sperm, and occasionally in addition to sperm donation. It can be used in teratozoospermia, beside azoospermia / oligospermia; because once the egg is fertilized, abnormal sperm morphology does not appear to influence Blastocyst development or Blastocyst morphology. Even with severe teratozoospermia, microscopy can still detect the few sperm cells that have a "normal" morphology, allowing for optimal success rate.

For most men, a 2-3 day break is ideal. That gives the "sample" an opportunity to regenerate. Too "old" of a sample raises the risk of poor motility, white cells, and other problems. (An "old" sample would be that which is taken after more than 7 days of abstinence).

If you are trying to become pregnant or are already pregnant, it is safest to avoid raising your core body temperature and therefore it is advisable to refrain from using the hot tub.

Just don't overdo it. Air travel is fine as long as the pressure is maintained, which it generally is in commercial aircraft.

Infertility is the inability to conceive after a year of unprotected intercourse in women under 35, or after six months in women over 35, or the inability to carry a pregnancy to term.

Both male and female factors contribute to infertility. Some studies suggest that male and female factors contribute equally. In many cases it may not be possible to definitely explain the reasons for infertility. It is essential that both the male and female partners be evaluated during an infertility work up.

Many things can change a woman’s ability to have a baby. These include age, smoking, excess alcohol use, stress, poor diet, being overweight or underweight, sexually transmitted infections (STIs), health problems that cause hormonal changes, such as polycystic ovarian syndrome and primary ovarian insufficiency.

Most physicians advise you not to be concerned unless you have been trying to conceive for at least one year. If the female partner is over 30 years old, has a history of pelvic inflammatory disease, painful periods, recurrent miscarriage, or irregular periods, it might be prudent to seek help sooner. If the male partner has a known or suspected low sperm count, then it would also be prudent to seek help sooner than waiting a year.

For a normal couple, it often takes a number of perfectly timed cycles before pregnancy is achieved. The chances of getting pregnant each cycle decreases as you get older. If you are between 20-25, your chance per cycle is about 25%. At 25-30 your chances are about 20%. At 30-35 it is about 15%. After 35 it may be about 10% per ovulatory cycle, and the chances continue with a downward trend.
